As participating Medicare providers, we accept assignment of benefits and will file all insurance claims for you. You are responsible for full payment of any deductible, any noncovered services, and/or co-insurance at the time of service.

We will file all insurance claims for you. It is your responsibility to present your insurance card, referral form or number prior to service being rendered. It is your responsibility to contact your PCP if a referral is necessary to see a specialist provider, our oƯice will NOT obtain that referral on the patients’ behalf. It is also the patient's responsibility to call their insurance prior to their visit to confirm that our oƯice is in network with their insurance. We will bill all insurance but if we are not in network and we see you, you will be responsible for all balances.
